An Individualized Approach to Care

For the staff at the Vascular Center of Northern Michigan, a quality, personal experience for each patient is our priority. On your first appointment, we will collect your medical history and you will have a comprehensive evaluation with either Dr. Colburn, Dr. Boros, or Dr. Roos.

For your visit, please bring the following:

• Completed history and registration forms that can either be mailed to you or printed from our website.  Print our New Patient Packet here.
• All medical insurance cards and necessary referral forms.
• Any medication lists and previous surgery lists
